Job Summary

At Ekklesia Care, we are committed to delivering client- and family-centered care. Our Registered Nurses and Registered Practical Nurses play a key role in providing and coordinating home health nursing services for individuals, families, and groups. Service s are delivered according to the College of Nurses of Ontario (CNO) Practice Standards, internal policies and procedures, program guidelines, and the Community Health Nursing Standards of Practice.

Key Responsibilities

As an RN or RPN, your responsibilities will include, but are not limited to:

  • Conducting client assessments and ongoing care planning and coordination
  • Monitoring and evaluating care plan interventions based on reassessment outcomes
  • Maintaining a high standard of care through sound clinical judgment and professional autonomy
  • Demonstrating leadership, accountability, and professionalism in interactions with clients and families
  • Supporting safe, quality care in alignment with continuous quality improvement principles
  • Communicating client updates, concerns, and changes to the nursing supervisor and/or case coordinator, and documenting accordingly
  • Ensuring all documentation complies with CNO standards

Qualifications

  • Registered and in good standing with the College of Nurses of Ontario
  • RN diploma/degree or RPN diploma from a recognized institution
  • Valid First Aid and CPR certification
  • Wound care and venipuncture experience is an asset
  • Experience in acute care and/or medical-surgical nursing is an asset
  • Community health nursing experience preferred
  • Strong written and verbal communication and documentation skills
  • Reliable, punctual, and available for scheduled shifts
  • Demonstrates critical thinking, commitment to professional development, and lifelong learning
  • Valid Ontario G driver’s license and access to a reliable vehicle (for community visits)
  • Current Vulnerable Sector Check
  • Up-to-date immunization record, including TB test within the last 6 months
  • Able to work both independently and collaboratively as part of a team
